To be eligible to enroll in UHG, an Employer Group may be a sole proprietor with employees, corporation or limited liability corporation with employees and there must be a minimum of two (2) to a maximum of fifty (50) eligible full-time employees (working 20 hours or more) on the effective date of the original enrollment. There is a minimum participation requirement based upon the number of eligible full-time employees. Additional eligibility requirements include:


• An Employer is required to conduct business within the State of Arizona for a minimum of sixty (60) days before enrolling in UHG coverage.

• Due to the bare period legislation requirements, an enrolling business must be uninsured and have been uninsured as a business for over 90 days.
